The links to the required application forms are in our documents section of the website. We highly recommend that you read through the 2023-2024 Parent-Student Handbook before proceeding to fill out and turn in the rest of the forms. An update to the handbook will be posted prior to the new school year. Once completed, the forms may be dropped off to the office.

Along with the school forms, families will be required to submit for each student a birth certificate, updated immunization records, previous school records, and required financial information if applying for tuition assistance or state voucher assistance.
